The Main Medicare Cost Categories

Part A

But Part A can still involve deductibles and coinsurance.

Part B

Part B covers physician services, outpatient hospital services, certain home health services, durable medical equipment, and other medical services not covered by Part A. CMS lists the 2026 standard Part B premium at $202.90 and deductible at $283.

After the deductible, Medicare generally lists 20% coinsurance for many covered Part B services when assignment applies.

Part D

Part D costs vary by plan. Medicare says no Part D plan may have a deductible over $615 in 2026, and some plans have no deductible.

For covered Part D drugs, Medicare says out-of-pocket spending reaches catastrophic coverage at $2,100 in 2026, after which there is no out-of-pocket cost for covered Part D drugs for the rest of the calendar year.

IRMAA

Higher-income beneficiaries may pay income-related monthly adjustment amounts for Part B and Part D. Medicare 2026 cost publications list Part D income-related adjustment amounts and state that the 2026 Part D national base premium is $38.99, used to estimate the Part D late enrollment penalty and income-related adjustment amounts.
